We want you to feel at home, that's why the house looks exactly like... a home! Your new home will be finished to a high standard and furnished in a contemporary style. You'll have your own room, it will come in neutral decor, but if you want to paint it purple or stick posters on the wall, you can. There's a modern kitchen perfect for cooking and eating together. There's a sitting room where you can hang out with friends, and an outdoor space that's perfect for BBQ's.
Your home will be staffed 24 hours a day. Having a small, dedicated support team means that everyone will get to know you as an individual. At all times you will be listened to and you can choose how to spend your time. We'll get to know your likes and dislikes, which means that as well as being able to help with the big things like further education choices, we'll also remember if you don't like peas.
Your social life is important to us and that's why we'll work closely with you to develop a social activity programme that will incorporate your current hobbies and interests. We'll also introduce some new ones, to enable you to reach your long term goals and ambitions.
There's an abundance of nearby leisure facilities that include swimming, horse riding, football, basketball, rock climbing, art and crafts, cookery clubs, pubs, cinema, theatre, religious activities and nightclubs to name a few. But the list doesn't end there - we also have access to Music teachers and Therapists. We'll help you plan holidays and short breaks with your friends. And there's plenty of local college courses you might like the look of, such as: woodwork, relaxation, health and beauty care and life skills. Oh, and not forgetting night school classes. Don't forget that although we'll always be there to help you go rock climbing, if you just want a night in in front of the telly with a take-away, we'll sort that out too.
Our supported living services aim to support young adults with learning disabilities to live in ordinary accommodation of their own by providing them with individualized packages of support.
We'll work together with you to develop an individually tailored support plan that will revolve around you. We'll listen to you and find out how you want to be supported to live your life, and help you to think about what you want now, and in the future. Embark's support staff, your family, friends and local authorities will then work together with you, to make it happen.
The provision of our support package includes the help and supervision with the following tasks:
Embark can also help you through every process of your tenancy. The tenancy is wholly independent of the support provided by Embark, thereby ensuring a healthy separation between accommodation and care.
Embark will take referrals from a variety of sources, including parents, individuals themselves, advocates, social workers, care managers and learning disability support teams. Referring someone to Embark is simple; all it takes is a telephone call. Once a referral has been made, we will strive to ensure that an appointment is made within one week of the referral date. We will then visit you, in an environment of your choice, and at a time suited to you. This meeting will give us time to discuss initial needs and will give you the chance to put forward any questions you may have.
If you are interested in using Embark, you will then be given the opportunity to develop and participate in a plan for your future. This plan will map out what you require from the service. At this point it is necessary to inform your social worker of Embark's involvement. This will also enable the social worker to assess your needs for the service.
Once a person-centred plan has been developed, and you have a current local authority assessment and a funding agreement with the local authority, Embark will begin to design a bespoke support package to meet your individual needs. You will then be invited to become involved in the recruitment of your staff team.
